AUTHORIZATION FOR SECURITY PATROL INSPECTIONS

OFFERED T0 THOSE WITH �ENHANCED� MEMBERSHIP ONLY

Mid May 2009 thru Mid May 2010

We want to standardize this service so you know what to expect.  By signing below, you are authorizing on-site inspections of your
Cascade Shores home for any obvious problems including storm damage, running water, break-in, unlocked or open doors & windows, etc.
The patrol officer will normally inspect on foot and walk around the perimeter of your house.  Under tough weather conditions or if there are
access problems, he will do the best he can.  Our volunteer officer is Mike Inglis who lives in Cascade Shores, knows the area, and reports to
the board.  If Mike is unavailable, the service will be performed by a substitute, most likely a board member.  Our officer is unpaid but is
reimbursed for vehicle expenses.  These forms will be given to Mike for his exclusive use.  Mike Inglis can be reached at 265-9147 for any questions.

SECURITY PATROL INSPECTIONS MAY INCLUDE ANY OF THESE  (Please circle the service(s) you want):

1. Vacation Inspections, 2 times per week of absence.  (Notify Mike at least two   weeks in advance of your departure.)

2. Unoccupied Home Inspections, 1 time per week.  This is for the members who have homes or vacation cabins that are kept unoccupied most of the time.

If you are requesting �Unoccupied Home Inspections,� please provide the following out-of-area contact information so that you
can be called about any problems discovered from inspections.

3.  Occupied Home Drive-by Checks, 1 time per week.  Unlike 1&2 above, this is a less intrusive inspection designed for members who
would like the comfort of the officer checking on their homes even though they are full-time residents.  For this service, he will simply
make a visual inspection from the end of the driveway but will not go up to the home or get out of his vehicle.
